1. Acceptance of these Terms

By accessing or using this website, you agree to these Terms to the extent permitted by applicable law. If you do not agree, please do not use the site. Acknowledging these Terms on a form is not consent to receive marketing calls, automated or prerecorded calls, or text messages. Any text-message consent is requested separately, is optional, and is not a condition of purchase.

2. Reliant Health Solutions' role

Reliant Health Solutions is an insurance agency, not an insurance company. We help consumers review insurance options.

• Insurance policies are issued and administered by insurance carriers, not by Reliant.
• Eligibility decisions are made by the applicable insurer or other authorized entity.
• Premiums, policy terms, benefits, limitations, exclusions, provider networks, formularies, and coverage conditions are set by the applicable insurer or program.
• We cannot guarantee that you will qualify for a particular plan or that a plan will remain available.
• We cannot change the contractual terms of an insurance policy.

3. Licensing and where services are available

Insurance activities are conducted subject to applicable licensing requirements and insurance laws in the jurisdictions where Reliant and its insurance professionals operate. Insurance products and services are available only in jurisdictions where Reliant Health Solutions and/or the applicable insurance professional is appropriately licensed or authorized. Not all products, carriers, plan types, or services are available in every state.

4. Website information is not a quote or offer

General website content, marketing information, and preliminary information are different from an actual carrier quote, an insurance application, and an issued policy. Nothing on this website is by itself:

• A formal insurance quote, an offer to insure, or an insurance contract.
• A guarantee of eligibility, premium, or benefits.
• A guarantee that a doctor, hospital, pharmacy, or medication is in network or covered.

Provider networks and drug formularies change. Please verify current participation and coverage directly with the applicable carrier or official plan documents before relying on it.

5. The issued policy controls

If website content, marketing materials, summaries, emails, or informal explanations conflict with an issued insurance policy or other official carrier document, the policy or official document governs to the extent provided by applicable law. Nothing in these Terms limits rights you have under applicable insurance or consumer-protection law.

6. Forms you submit

Submitting a quote request, contact, or careers form lets us respond to your request. It does not by itself create an insurance contract, create or bind coverage, guarantee a quote, eligibility, or acceptance, or create an insurer-insured relationship. Please provide accurate information and do not submit sensitive information such as Social Security numbers, payment details, or medical records.

7. Phone, email, text, and electronic communications

When you submit a form or contact us electronically, we may respond using the contact details you provide for purposes related to your request. We send text messages only if you check an optional text-message box on a form, and marketing texts only if you check the separate marketing box. You can reply STOP to any text or email us to stop communications. Entering a phone number does not give us unlimited permission to call or text you.

Using this website is not an electronic signature on an insurance application, and a website form does not replace any legally required insurance document or disclosure. Any electronic records or signatures used in an actual insurance transaction will be handled as applicable law, including New York's Electronic Signatures and Records Act and the federal E-SIGN Act, requires.

8. Producer compensation

Insurance professionals are generally compensated by insurers when coverage is placed. Any compensation disclosure required for an insurance transaction will be provided separately as applicable law requires; these Terms are not a substitute for that disclosure. 9. Carrier names and trademarks

Carrier names, logos, trademarks, service marks, and plan names belong to their respective owners. Reliant does not own them, and their appearance on this site does not mean that a carrier endorses Reliant or this website.

10. No government affiliation

Reliant Health Solutions is a private insurance agency. We are not New York State of Health, Medicare, Medicaid, the Centers for Medicare & Medicaid Services, New York State, the federal government, or any other government agency or marketplace, and we are not affiliated with or endorsed by any of them.

11. Not medical, legal, tax, or financial advice

General website content is educational and is not medical, legal, tax, or financial advice. Please consult an appropriate professional on those subjects. This does not limit the insurance guidance our licensed professionals may provide.

12. Accuracy and updates

We make reasonable efforts to keep information accurate, but premiums, eligibility, carrier participation, plan availability, benefits, networks, formularies, enrollment rules, government program information, subsidies, tax credits, and deadlines change often. Please confirm information with a licensed professional or the carrier before you make an insurance decision.

13. Careers

Submitting an application does not guarantee employment. Job listings may be changed or withdrawn, applications are reviewed separately, and nothing on this website creates an employment contract.

14. Acceptable use

You agree not to:

• Use the site for unlawful, fraudulent, or infringing purposes.
• Submit false information or impersonate someone else.

• Upload malicious software.

Screen readers, keyboard navigation, browser zoom, other assistive technology, ordinary search engines, and normal browser features are always permitted.

15. Intellectual property

Our original website copy, design, graphics, branding, and original images are owned by Reliant Health Solutions or its licensors. You may view and print pages for personal, non-commercial use. We do not claim ownership of carrier trademarks, government marks, licensed stock material, or other third-party content.

This site may link to insurance carriers, provider directories, government websites, enrollment resources, and other third-party tools. We do not control their availability, security, content, terms, or privacy practices. This does not limit our responsibility for our own statements about a third-party product.

17. Privacy

How we handle personal information is described in our Privacy Policy, Cookie Policy, and Financial Privacy Notice. This website is intended for adults. A parent or guardian may provide information about dependents as part of an insurance inquiry.

18. Website availability

We may modify, suspend, maintain, or discontinue any part of the website and do not promise uninterrupted or error-free access.

19. Disclaimer of warranties

22. Other agreements

These Terms govern use of the public website only. They do not replace insurance policies, applications, certificates, enrollment documents, carrier agreements, required disclosures, financial privacy notices, or other agreements governing an insurance transaction. If a separate agreement applies to a service, it controls that service.

23. Changes, severability, and no waiver

We may update these Terms going forward and will change the "Last updated" date above. For material changes we will provide reasonable notice on the website where appropriate. If any provision is found unenforceable, the rest continue to apply to the extent legally permitted. Not enforcing a provision once does not waive our right to enforce it later.
